CHAOS Marine Holdings LLC

PROMISES, GENERAL RELEASE, WAIVER, AND CONSENT AGREEMENT

As the parent or legal guardian of the child registered, I hereby give my full consent and approval for my child to participate in Camp CHAOS.

I understand that my child, is boarding a charter fishing vessel to fish open water. I understand and I am willing to assume the risks and hazards associated with offshore fishing on behalf of my child. These risks and hazards include, but are not limited to sun, rain, lightning, sea spray, and any other weather conditions; sea sickness; pitching and rolling; slippery and wet decks; injuries, internal or external, from: boarding and disembarking the vessel; fish, fishing, fishing tackle, and gear; and the unintentional conduct of any other persons on the boat.

In addition to giving my full consent for my child’s participation, I do hereby waive, release and hold harmless the Captain, Crew, and CHAOS Marine Holdings, LLC for risks/hazards associated with offshore fishing. I further understand that this trip is not recommended for anyone with preexisting conditions such as circulatory problems; high blood pressure; heart conditions; pregnancy; weak bones; knees, back or ankle problems.

I have read this Agreement and understand it is a legal document designed to inform me of risks and release the Captain, Crew and Corporation from any and all liability for personal injuries, mishaps, or omissions from the conduct of other fisherman (intentional or not) and crew (unintentional) during this fishing charter. I agree that my signature on the page constitutes a waiver and release for any future charters the child named above may take with CHAOS Marine Holdings LLC.

I consent for the child to receive medical treatment, which may be deemed advisable in the event of injury, accident and/or illness during this event.

I understand that at this event or related activities the child may be photographed. I agree to allow my child’s photo, video or film likeness to be used for any legitimate purpose by the charter operator.

The Accident Waiver and Release of Liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law. I hereby certify that I have read this document and I understand its contents.
